Preoperative Trans-Arterial Embolization and Surgical Resection for Giant Liver Hemangioma: A Multidisciplinary Approach

چکیده
A 47-year-old male presenting with abdominal distension and pain was found to have a giant hemangioma of the liver measuring 22cm in its greatest diameter upon preoperative MRI. The patient underwent preoperative trans-arterial embolization (TAE) of the hemangioma, followed by extended right hepatectomy for definitive treatment. This case illustrates a novel multidisciplinary approach to the treatment of a massive liver hemangioma. We propose a new classification of these > 20 cm hemangiomas that require preoperative TAE as “Super Hemangiomas.”

Pre-operative arterial embolization of symptomatic giant hemangioma of the liver.
The cavernous hemangiomas of the liver are usually small sized and asymptomatic. Most of them are incidentally diagnosed and a very small portion requires therapy. Giant hemangioma can be symptomatic, and this condition is the indication for treatment. The striking complication of surgical treatment of cavernous hemangiomas is intra-operative blee...
متن کاملSurgical Management for Giant liver Hemangiomas Greater Than 20 cm in Size
BACKGROUND/AIMS The aim of this study was to investigate the primary management experience for giant liver hemangiomas greater than 20 cm in size. METHODS Records of patients referred for evaluation of radiologically and/or histopathologically proven giant liver hemangiomas between January 2007 and March 2010 were retrospectively analyzed. The reasons for referral, results of imaging studies,...

BACKGROUND Hepatic hemangioma is a congenital tumor of the mesenchymal tissues of the liver. While typically benign, these tumors can occasionally grow to sufficient size to cause a number of symptoms, including pain, severe hepatic dysfunction, or, rarely, consumptive coagulopathy. In such instances, surgical treatment may be warranted. CASE REPORT We present a case of a symptomatic giant he...
متن کاملSpontaneous rupture of a giant hepatic hemangioma. Sequential treatment with preoperative transcatheter arterial embolization and conservative hepatectomy.
Hemangioma is the most common benign tumor of the liver and it is often asymptomatic. Spontaneous rupture of liver hemangiomas is a rare but potentially lethal complication. Emergent hepatic resection has been the treatment of choice but carries high operative morbidity and mortality. Recently, preoperative transcatheter arterial embolization (TAE) has been used successfully for the management ...
متن کاملLeft hemihepatectomy and caudate resection after transarterial embolization for multiple giant liver hemangioma
Hemangiomas are the most common benign tumors of the liver and need to be treated in cases where they are accompanied with symptoms, have a risk of rupture, or are hardly distinguishable from malignancy. The authors adopted Transarterial embolization preoperatively to alleviate progressive symptom and avoid rupture of tumor, using staged resection method instead of liver transplantation, and pr...
